Diabetes Specialist Nurse, Band 7

Salary £31,383 - £41,373 per annum + HCAS £4,483 per annum

This is an exciting opportunity to work as part of the multi-disciplinary Diabetes Services with Bromley Healthcare.

We are looking for a committed, forward thinking and enthusiastic team player with energy, commitment and drive, who wants to join an innovative Social Enterprise and who will embrace the Bromley Healthcare ethos, to provide specialist care to patients within the Diabetes Service. This will include acting as a role model for other staff, sharing learning and developing practice.

We currently provide community/diabetes education services across Bromley and Croydon, and are actively looking to expand on this portfolio.

This is an opportunity for someone with an enthusiasm for diabetes and a wish to extend their expertise in this field to be part of a team of dedicated healthcare professionals.. The post will include responsibility for a mixed caseload including people with Type 1and Complex Type 2. You will provide comprehensive, evidence based diabetes care for people with diabetes and support healthcare professionals both within the Community and General Practice clinics within the Bromley and Croydon areas. You will be required to work as a highly skilled practitioner implementing treatment based on expert clinical practice for both people with diabetes and other professionals providing care, in a range of settings.

You will need to be confident in delivering education to groups and ideally have experience of structured educations programs e.g. DESMOND/ DAFNE. You will be involved in the development of Diabetes within the Bromley Healthcare Diabetes Services. You will need excellent clinical, organisational, IT and communication skills. Car driver essential.

During 2014/15 diabetes services have undergone a whole system transformational change, a programme which is still on-going and the post holder will play a vital role in supporting these changes. This pioneering new service provides an integrated service across primary, community and secondary care services. Bromley Healthcare is now the prime contractor for the delivery of these services.

We are working to develop more routine and specialist clinical services in community and primary care moving care away from the hospital setting as well as enhancing clinical skills in primary care.

We work in partnership with King's College Hospital to provide consultant support in community and primary care. The post holder will also support the continued development of primary care diabetes services through our Advanced Primary Care Practice scheme which has been highlighted as one of key successes of the changes.
