Advance Nurse Practitioner (Older People’s Care) Intermediate Care Team (IRC17483)

Band 7 – Salary Range £32,974 - £43,471 per annum

Plus Distant Islands Allowance of £1,738 per annum

Plus Relocation Assistance of up to £8000 available

We seek to appoint an experienced Advanced Nurse Practitioner (ANP) to work as part of the Intermediate Care Team.

Following the successful implementation of an Intermediate Care Team, we aim to expand the services offered through the team by supporting early assessment, planning and treatment for individuals who become unwell at home and require a rapid response in order to maintain them at home.

As an Advanced Nurse Practitioner you will see individuals with undifferentiated and undiagnosed problems. Based on patient history, clinical assessment and a comprehensive physical examination, utilising expert knowledge and clinical judgement, you will be able to identify the potential diagnosis. You will be able to initiate laboratory and radiological investigations, make referrals, prescribe and evaluate treatments appropriate to the clinical situation and needs of the patient. You will also be able to assess, plan and implement programmes of care, in liaison with multidisciplinary colleagues, to support the individual to remain at home with appropriate supportive measures thus avoiding unnecessary hospital admissions.

As an experienced nurse with Registration on Part 1 of the NMC Register, you will also possess extensive post registration experience working in Adult/Older People’s care and either possess a Nurse Practitioner Qualification or be expected to have completed / working towards a Masters degree in Advanced Practice which incorporates a programme of Advanced Clinical Examination Skills. Candidates are also expected to possess an Extended Independent (non medical) Prescribing qualification or be willing to undertake this programme of study. Evidence of ongoing professional development is essential. You will also be expected to be competent in a range of clinical skills to support the provision of nursing care in the home environment, including the ability to manage palliative care. Good communication, interpersonal and computer skills, together with an awareness of community legislation are essential. A full driving license is also required.

This post will be based within the Intermediate Care Team but will work closely with the Community Nursing service and the wider Multi-disciplinary team based in Lerwick to support the health and care needs of individuals both resident in their own home and within residential care homes within the community.

This is an exciting time to join the staff of NHS Shetland, where we are increasing our number of Advanced Nurse Practitioner (ANP) posts across the service to support the ongoing development of a range of patient services. Appropriate support and supervision arrangements are in place from senior colleagues, designated General Practitioners (GP’s) and from the Advanced Nurse Practitioner (Practice Education).

Informal enquiries are welcome. Please contact Edna Mary Watson, Chief Nurse (Community) on 01595 743377.

Closing date: Friday 28 September 2018

Interviews will be held in Shetland during October 2018.
